Character Background Questionnaire

Hey All,

When I ran 2nd Edition, I seem to recall a two page questionnaire for players to fill out and provide background info for their characters. I can't remember if this was something I photocopied out of one of the books, or if I downloaded it from somewhere. Either way, does anyone have a link where I can obtain such questionnaire?

Here's the one my best friend uses for his games:

What is your character's name?
What is your character's race?
What sex is your character?
What is your character's discipline?
What is your character's height?

What is your character's weight?
What is your character's eye color?
What is your character's hair color?
What is your character's skin color?
How old is your character?
Where was your character born?
What is your character's general appearance? (Include clothing styles)
Does your character have a family? How many?
How do they feel about your lifestyle?
Does your character want a family?

What are your character's feelings about:

Look at that ... I appears what I was looking for is Step 9 of character creation in 4e (p. 72). It's just not in a nice printable form like previous editions.
