The other significant bump in the road is at Step 19, where the d20 is added. The Variance increases by almost 50%, so while the Expected Value is 19 as it should be, the dice are much more likely to fail spectacularly (and succeed spectacularly). Correct is:
output 5d[explode d4]
Every four is rerolled
I failed with
output [explode 5d4]
Only when all dice show a four there is a retoll.

This is intentional, and IMO has been since the original version of the system.
